In order to rent a mailbox, you need to complete the US Postal Form 1583 for all box occupants.  If you want to download the form, it is available in Adobe PDF format (requires the reader to be installed on your computer.)

If you are completing it to fax into us, it requires that it be notarized and signed in the presence of a notary.  Once faxed in, we will establish contact with you to accept payment, and advise you of your box number at that time!
